SCAI Expert Consensus Statement on Management of In-Stent Restenosis and Stent Thrombosis

چکیده
Stent failure remains the major drawback to use of coronary stents as a revascularization strategy. Recent advances in imaging have substantially improved our understanding mechanisms underlying these occurrences, which common numerous clinical risk factors and mechanical elements at time stent implantation. In-stent restenosis problem despite improvements in-stent design polymer coatings over past 2 decades. It generates significant health care cost is associated with an increased death rehospitalization. thrombosis causes abrupt closure stented artery therefore carries high myocardial infarction death. This Society for Cardiovascular Angiography & Interventions (SCAI) Expert Consensus Statement suggests updated practical algorithmic approaches thrombosis. A pragmatic outline assessment management patients presenting presented. new SCAI classification that time-sensitive mechanistic implications proposed. Emphasis placed on frequent intracoronary timing determine precise etiology because information crucial guide selection best treatment option. recommends image-guided stenting initial implantation minimize occurrence failure. When are encountered, should be strongly considered optimize subsequent approach.

Overview of Intracoronary Brachytherapy for in-Stent Restenosis of a Drug-Eluting Stent
Percutaneous coronary intervention with stenting is considered recently as the most common procedure for the treatment of symptomatic coronary. The article reviewed 41 studies published during 1997-2019 on intracoronary brachytherapy for in-stent restenosis of a drug-eluting stent. Intracoronary radiation therapy was finally confirmed in the setting of in-stent restenosis using as adjunctive th...
متن کاملInfluence of residual stenosis after coronary stent implantation on development of restenosis and stent thrombosis.
Sick et al,1 report in a retrospective study on a selected population (only patients who underwent angiographic follow-up) the lack of any statistical significant association between stent overdilatatation, expressed as negative postprocedural stenosis, and the occurrence of thrombosis or stent restenosis. The authors conclude that ticlopidine treatment is the only condition to lower the risk o...
متن کاملSCAI expert consensus statement for infrapopliteal arterial intervention appropriate use.
Intervention to treat infrapopliteal arterial disease can be challenging because the patients' comorbidities, the anatomic variables, and the limitations of our techniques. Clinical scenarios based on anatomic and clinical variables are presented. Recommendations regarding intervention (appropriate care, may be appropriate care, rarely appropriate care) are made based on best evidence.
متن کاملSCAI expert consensus statement for renal artery stenting appropriate use.
The pathophysiology of atherosclerotic renal artery stenosis (RAS) includes activation of the renin-angiotensin-aldosterone axis with resultant renovascular hypertension. Renal artery stenting has emerged as the primary revascularization strategy in most patients with hemodynamically significant atherosclerotic RAS. Despite the frequency with which hemodynamically significant RAS is observed an...
متن کاملLipoprotein(a) and coronary thrombosis and restenosis after stent placement.
OBJECTIVES The objective of this prospective study was to evaluate the relation between high lipoprotein(a) levels and thrombotic and restenotic events after coronary stent implantation. BACKGROUND Lipoprotein(a) may promote atherogenesis, coronary thrombosis and restenosis after balloon angioplasty, but the clinical significance remains unclear. METHODS The study included 2,223 consecutive...
